What to Bring to Your First Appointment with Mr Patten
Please bring the following to your appointment (or send to us prior):
A referral letter from your doctor or medical specialist
Completed patient registration form. We will send this to you, or you can alternatively download the form and complete it below
Medicare, private health insurance and/or DVA card/s, if applicable
Relevant radiology results, if required. At the time of booking your appointment, please inform us of where your imaging was performed so that we can ensure we are able to access your results online.
Consultation Fees
Initial consultation - standard: $280.00
Review consultation: $165.00
Initial consultation - second opinion or complex condition: $350.00
Cancelling an Appointment
If you are unable to attend your appointment, please contact us as soon as possible so that we can offer your cancelled appointment to a patient on our waiting list. We ask that you kindly provide at least one business day’s notice when cancelling your appointment.
