What to Bring to Your First Appointment with Mr Patten
Please bring the following to your appointment (or send to us prior):
Doctor's referral letter from your GP or medical specialist involved in your care.
Completed patient registration form. We will provide you with this or you can download the form below.
Any relevant radiology (x-ray, MRI, bone scan, etc) reports in your possession.
An up-to-date list of your current medications.
Any previous letters or documentation related to your condition.
Consultation Fees
Initial consultation: $250.00
Review consultation: $150.00
Both fees attract a Medicare rebate, which we can claim on your behalf electronically at the time of payment, provided you have a valid referral in place from your doctor. Payment is required on the day of your appointment. We accept payment via EFTPOS or cash.
Cancelling an Appointment
If you are unable to attend your appointment, please contact us as soon as possible so that we can reschedule this and offer your cancelled appointment to another patient. We ask that you kindly give us at least 24 hours’ notice, where possible, when cancelling an appointment.
